To Make an Appointment:
Please call us at 503-777-8999 to set up an appointment. Please have your Primary Care Veterinarian send a copy of your pet’s medical records. All first time patients should download and complete our New Patient Forms (see orange panel to the right) and bring them with you for your first appointment. Also, please review the First Speciality Visit list below.

Referring Veterinarians Who’d Like to Find Out More About ADVRC:
Please call us at 503-777-8999 and we’d be happy to send you more information about our practice and even schedule a tour of our facility. Please inquire about our annual dermatology conference, The Annual Tri-State Veterinary Dermatology Conference, held in Portland, Oregon in the Fall of every year.

First Specialty Visit:
1)   Please do not bathe your pet 3 days prior to your appointment.
2)   Please do not apply any topical medications to your pet’s skin a minimum of 3 days prior to your appointment (call about discontinuing certain medications which might interfere with any tests).
3)   Please do not feed your pet after midnight on the evening prior to your appointment (please call for instructions if your pet has diabetes). Your pet may have free access to water.
4)   When scheduling your appointment, please provide our office with information from all veterinarians that have previously treated your pet. This will allow us to evaluate previous testing and treatment.
5)   Please bring a list of medications, shampoos and food that your pet is currently using. If it is easier, feel free to bring these items with you.
6)   Please arrive fifteen minutes early for your appointment. You will be asked to complete a short form detailing your pet’s medical history that will help us create your pet’s medical record.
7)   Please allow 1 to 2 hours for your first appointment to allow time to review your pet’s medical history, give a thorough examination, and discuss the diagnosis and treatment with you. In some cases we may need your pet to stay with us for several more hours for evaluation.
8)   To find the best solution for the long-term health of your pet, your Primary Care Veterinarian receives a letter summarizing your visit. This ensures clear communication between your Primary Care Veterinarian and The Allergy and Dermatology Veterinary Referral Center.
